Cone Health Helps Make Post-Hospital Care Arrangements Easier

Cone Health and Total Living Choices, a healthcare technology company, have joined to provide patients and family members with an easy-to-use tool for reviewing their options and making good decisions for post-hospital care.

“Whether patients are coordinating care locally or looking for a facility or home healthcare service miles away, our comprehensive, complimentary online resource places the needed information at their fingertips. For the first time this allows true participation for our patients and their families in after-hospital care,” says Annette Smith, RN, Vice President, Nursing and Patient Services, Wesley Long Hospital. “For example, before they even come into the hospital for surgery, our joint replacement patients can arrange for a walker or hospital bed to be at home once they are discharged from the hospital.”

Cone Health is the first healthcare network in North Carolina to partner with Total Living Choices to help people find assisted living, home health or some other form of after-hospital care.The Care Finder Pro® system will also be used by Cone Health staff to make care arrangements for patients. Total Living Choices President and CEO Kyle Salem says that the partnership will enable Total Living Choices and Cone Health to educate patients on available providers in their time of need.

“We believe patients should have access to every available resource. Our goal is to help unburden hospitals from tedious paperwork so they can focus on what really matters – the patients,” Salem says.
