Cone Health.

“My goal is to help coordinate the efforts of our health system, our THN providers and various community resources to improve the health of our entire community,” says Osborne.

Population health refers to nationally recognized efforts that THN and Cone Health take to create healthier communities: helping healthy people stay healthy and helping people with chronic illness stay stable. Examples of these efforts include ensuring that patients are up to date on vaccinations and preventative health screenings, showing people how to better clean their homes to reduce asthma attacks, and ensuring that those with chronic disease have and know how to take their medications. The goal is to help people stay healthy and to avoid expensive hospital stays.

Osborne previously served as THN’s primary care medical director, a position he held since September 2015. He is a graduate of the University of North Carolina School of Medicine. For 29 years, he practiced internal medicine at Eagle Internal Medicine @ Tannenbaum. He now practices sleep medicine at the Eagle Sleep Center in Greensboro. He will remain there in a part-time capacity.
