Sickle Cell Support and Resources
Our distinction is in our comprehensive, holistic and dedicated care for people living with sickle cell. This includes extensive support and resources you can access for the best treatment and care of sickle cell.
A dedicated Patient Navigator/Social Worker navigates the patient through the treatment and care at the Sickle Cell Medical Center. They provide psychosocial assessment and support for the patient.
Our navigator has access to additional resources to assist with mental health issues, crisis counseling, advanced directives, nursing home or residential hospice placement, and many other non-medical needs.
Cone Health partners with the Piedmont Health Services and Sickle Cell Agency and works with its Community Case Manager to provide you with access to additional community support.
Your Nurse Case Manager is the point person for any and all questions and concerns about medical needs. In this role, the manager provides patient care while in the acute care setting and then assists in the transition to self-home care, hospitalization or long-term acute care. The Nurse Case Manager can also assist with information about referrals to other recourses within the Cone Health network of care.
