Congregational & Community Nurse Program

Since 1998, registered nurses in the Congregational and Community Nurse Program (CCNP) have served as Faith Community Nurses (FCNs), a recognized specialty by the American Nursing Association. These nurses support people of all faiths and community organizations. To serve, they must be registered nurses with an active license and BLS certification.

Cone Health’s CCNP works to close healthcare gaps and promote health equity. These nurses go beyond hospitals and clinics, bringing care directly to congregations and communities. They focus on overall well-being—body, mind, and spirit—while addressing health disparities.

Faith Community Nurse Role

  • Offer one-on-one health support
  • Help with basic needs like transportation, housing,
    food, and medicine
  • Connect people with doctors and hospitals
  • Guide patients through the health care system
  • Teach ways to stay healthy and manage illnesses
  • Support overall well-being, including mental and
    physical health
  • Check blood pressure, blood sugar, and other key
    health signs
  • Help prevent and manage high blood pressure
    and diabetes
  • Organize free health check-ups and flu shot clinics
  • Help people find a regular doctor
  • Reduce unnecessary ER visits
  • Prevent repeat hospital stays
