Danielle Phelps Swartz, MHA, BSN, RN, CCM, Vice President of Population Health at Cone Health, ensures that the various strategies and initiatives of the Value-Based Care Institute are implemented and optimized.
Phelps Swartz is a recognized leader in population health strategy and innovation. She plays a pivotal role in advancing Cone Health’s position as a regional leader in value-based care.
Phelps Swartz joined Cone Health in 2024 from UpStream, where she was chief nursing officer and senior vice president of care management. Her background includes responsibility for national strategies and programs aimed at improving the health and lowering costs for all populations.
Phelps Swartz earned a master’s in health care administration from Walden University and a Bachelor of Science degree in nursing from Duquesne University in Pittsburgh, Pennsylvania.
Phelps Swartz is dedicated to developing future leaders serving as an advisor for the Women’s Leadership Program at the University of Tennessee, Knoxville.
Phelps Swartz is on the boards of Cone Health Ventures and HealthTeam Advantage.
