Cone Health requires the COVID-19 vaccination for all employees, medical and dental staff, professional students and volunteers. All new hires are required to receive the first dose of the COVID-19 vaccine series (either the Moderna or Pfizer vaccine or the single dose Johnson & Johnson vaccine) prior to their first day of employment. New hires must receive the second dose (either the Moderna or Pfizer vaccine) within 35 days of their first day of employment. This is a condition of employment, unless a religious or medical exemption is approved. Appropriate documentation must be submitted within the designated timeframes once an offer of employment is made.
Bring all available immunization records and any available TB screening records to your appointment.
Immunization Requirements for Employees
CONE HEALTH POLICY
All new hires are required to receive the first dose of the COVID-19 vaccine series (either the Moderna or Pfizer vaccine or the single dose Johnson & Johnson vaccine) prior to their first day of employment. New hires must receive the second dose (either the Moderna or Pfizer vaccine) within 35 days of their first day of employment.
Mandatory unless a religious or medical exemption is approved.
Two doses for employees and affiliated individuals working in dietary services, dose at 0 month with second dose at 6 months. No titer required.
Mandatory: If declining this vaccine they must sign the declination form

One dose for those who are routinely exposed to isolates of N. meningitides (microbiology, pathology, autopsy). Booster vaccine every five years.
Measles (Rubeola), Mumps, Rubella
Two documented doses of MMR vaccine (no post titer required) or documented immune titer.
Tetanus/Diphtheria/ Pertussis (Tdap)
One adult dose for all staff.
One dose recommended every ten years after Tdap and after high-risk situations (bites, needle stick).
Two documented doses of varicella vaccine (no post titer required) or documented immune titer.
All new candidates must have a Quanitferon Gold Lab Assay and TB history review upon hire with Cone Health. Employees with positive results must complete appropriate follow up to rule out active TB including chest x-ray. Health Department referrals will be made in the county of residence for staff with positive Quantiferon Gold.

New candidate screenings are available by appointment through a Cone Health representative only. Our normal business hours are Monday thru Friday 8:00 a.m. – 4:30 p.m.
