Healthy Lifestyle Premium Program
IMPORTANT: Failure to complete the steps below and engage with this program will result in a medical insurance premium increase of $15 per pay period in 2020. If you are new to health insurance coverage after Jan. 1, 2019, you will need to get enrolled first.
Start Your Program
- Step 1: Employees and spouses on the health plan must get an annual physical between January 1, 2018 and July 1, 2019. Physical must be performed by a primary care physician or OB/GYN.
- Step 2: Employees and spouses need to complete at least one health action step by identifying their current health status and completing the matching health action step (please see below) between January 1, 2019 and September 1, 2019. *Failure to complete both of these steps will result in a medical insurance premium increase of $15 per pay period in 2020.
Stop at the first Health Status that applies to you. You only need to complete the 1st item that applies to you.
Enroll prior to 20 weeks and complete 75% of the program requirements Jan 1 –Sept 1
Actively participate in Wellsmith for at least 12 weeks or complete 3 Lifestyle and Condition Coaching calls.
*Additional calls or interactions may be required, based on the recommendation of your coach, to be considered engaged for the pharmacy benefit.

Telephonic Health Coaching with My Active Health
Complete 3 Lifestyle and Condition Coaching calls if you are diagnosed with Coronary Artery Disease, Chronic Kidney Disease, COPD, Congestive Health Failure, High Cholesterol, or Asthma.

Group Coaching with My Active Health
Participate in at least 3 Group Coaching calls. Call 1-855-294-6577 for more information or to enroll.
Complete the Quit Smart Tobacco Cessation Program. In person and telephonic options available.
Weight Management (BMI of 27.5 or higher or Prediabetes)
Complete the Weight Management program of your choice

New to health insurance coverage after Jan. 1, 2019?
Cone Health employees and spouses on a medical plan must first complete the “Get Enrolled Now” step which includes completing the health assessment at myactivehealth.com/conehealth website. New hires or status changes (ie. new to health insurance) must complete their health assessment within 30 days of enrolling in the medical plan.* Failure to complete this step will result in a $10 per pay period increase throughout 2019.
