2022 Healthy Lifestyle Program Info 


All employees and spouses on a Cone Health medical plan (Choice, Save, and Focus) on January 1, 2022 must complete the following to earn the Healthy Lifestyle Premium rate in 2023.

IMPORTANT: Failure to complete the steps below and engage with this program will result in a medical insurance premium increase of $15 per pay period.

If you are new to health insurance coverage (new employees or have a status change during the year) after Jan. 2, 2022, you will complete only your health assessment within 30 days of enrolling on a Cone Health Medical Plan.

Get Your Annual Physical

(Between January 1, 2021 and July 1, 2022)

Employees and spouses on the health plan must get an annual physical. Physical must be performed by a primary care physician or OB/GYN. Once completed, your Annual Physical will be automatically documented into your Active Health Rewards center after the insurance claim information has been received. Complete Your Health Assessment

(Between October 11, 2021 and September 1, 2022)

Employees and spouses must complete your health assessment at

Complete Your Action Step

(Between January 1, 2022 and September 1, 2022)

Employees and spouses need to complete at least one health action step by identifying their current health status and completing the matching health action step using the tool below. Don't Wait- Health action steps can take up to one month or more to complete.

New to health insurance coverage after Jan. 2, 2022?

Keep your premiums low in 2022!

Who: Employees and spouses who are new to a Cone Health medical plan (Choice, Save, and Focus).

What: Complete a Health Assessment to earn the Premium Discount rate in 2022.

By When: Within 30 days of coverage beginning on a Cone Health medical plan.

How: Create an account at and complete your health assessment.

New hires or status changes (i.e. new to health insurance) must complete their health assessment within 30 days of enrolling in the medical plan.* Failure to complete this step will result in a $15 per pay period increase throughout 2022.

The Healthy Lifestyle Premium Program is a voluntary wellness program for employees and spouses on the medical plan. If it is unreasonably difficult due to a medical condition for you to achieve the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, you may qualify for an opportunity to earn the same incentive through an alternative course of action, that will be developed with your doctor.
