Healthy Lifestyle Premium Program

Keep Your Premiums Low!

Who: All employees and spouses on a Cone Health medical plan (Choice, Save, and Focus) on or before January 1, 2020.

What: Must complete the following to earn the Healthy Lifestyle Premium rate in 2021.

IMPORTANT: Failure to complete the steps below and engage with this program will result in a medical insurance premium increase of $15 per pay period.

If you are new to health insurance coverage after Jan. 2, 2020, you will complete your health assessment within 30 days of enrolling on a Cone Health Medical Plan. Click here to learn more.

Between January 1, 2019 and July 1, 2020: Get your annual physical.

Employees and spouses on the health plan must get an annual physical. Physical must be performed by a primary care physician or OB/GYN. Once completed, your Annual Physical will be automatically documented into your Active Health Rewards center after the insurance claim information has been received. Between January 1, 2020 and September 1, 2020: Complete your health assessment.

Employees and spouses must complete your health assessment at www.myactivehealth.com/conehealth.

Between January 1, 2020 and September 1, 2020: Identify your current health status and complete the corresponding health action step.

Employees and spouses need to complete at least one health action step by identifying their current health status and completing the matching health action step in the chart below.

Stop at the first Health Status that applies to you. You only need to complete the 1st item that applies to you.

Enroll in the Diabetes Management Program with Wellsmith or Active Health Management

Actively participate in Wellsmith for at least 12 weeks or complete 3 Lifestyle and Condition Coaching calls.

*Additional calls or interactions may be required, based on the recommendation of your coach, to be considered engaged for the pharmacy benefit.

CAD, Chronic Kidney Disease, COPD, CHF, High Cholesterol, Asthma

Telephonic Health Coaching with My Active Health

Complete 3 Lifestyle and Condition Coaching calls if you are diagnosed with Coronary Artery Disease, Chronic Kidney Disease, COPD, Congestive Health Failure, High Cholesterol, or Asthma.

You can schedule an appointment with a coach by calling 1-855-294-6577.

Hypertension

Group Coaching with My Active Health OR Complete 3 "Under Pressure: Understanding Hypertension" classes.
Participate in at least 3 Group Coaching calls. Call 1-855-294-6577 for more information or to enroll.

Tobacco User

Register for and Complete the Tobacco Cessation Classes
Complete the Quit Smart Tobacco Cessation Program. In person and telephonic options available.

Weight Management (BMI of 27.5 or higher or Prediabetes)

Complete the Weight Management program of your choice
Enroll and actively participate in a designated weight management program if your BMI is 27.5 or higher or diagnosed as prediabetic.

No Status (Healthy)

New to health insurance coverage after Jan. 2, 2020?

Who: Employees and spouses who are new to a Cone Health medical plan (Choice, Save, and Focus).

What: Complete a Health Assessment to earn the Healthy Lifestyle Premium rate in 2020.

By When: Within 30 days of coverage beginning on a Cone Health medical plan.

How: Create an account at www.myactivehealth.com/conehealth and complete your health assessment.

New hires or status changes (ie. new to health insurance) must complete their health assessment within 30 days of enrolling in the medical plan.* Failure to complete this step will result in a $15 per pay period increase throughout 2020.

The Healthy Lifestyle Premium Program is a voluntary wellness program for employees and spouses on the medical plan. If it is unreasonably difficult due to a medical condition for you to achieve the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, you may qualify for an opportunity to earn the same incentive through an alternative course of action, that will be developed with your doctor.
