Healthy Lifestyle Premium Program

UPDATE: The 2020 Healthy Lifestyle Premium Program requirements have been waived. All plan members will receive the healthy lifestyle rate in 2021. Please start your requirements in January, 2021 to keep your premiums low for 2022.

KEEP YOUR PREMIUMS LOW! All employees and spouses on a Cone Health medical plan (Choice, Save, and Focus) on or before January 1, 2020 must complete the following to earn the Healthy Lifestyle Premium rate in 2021.

IMPORTANT: Failure to complete the steps below and engage with this program will result in a medical insurance premium increase of $15 per pay period.

If you are new to health insurance coverage after Jan. 2, 2020, you will complete your health assessment within 30 days of enrolling on a Cone Health Medical Plan. Click here to learn more.

STEP 1: Get Your Annual Physical

(Between January 1, 2019 and July 1, 2020)

UPDATE: Due to COVID-19 and the need to postpone routine annual physicals, the deadline for your annual physical has been extended to Sept. 1, 2020. You do not need to have your physical complete by Sept. 1, 2020, rescheduling your appointment by Sept. 1, 2020 will count as completion of this requirement, no matter when the actual physical is completed. Please send your rescheduled appointment information to livelifewell@conehealth.com.

Employees and spouses on the health plan must get an annual physical. Physical must be performed by a primary care physician or OB/GYN. Once completed, your Annual Physical will be automatically documented into your Active Health Rewards center after the insurance claim information has been received. STEP 2: Complete Your Health Assessment

(Between January 1, 2020 and September 1, 2020)

Employees and spouses must complete your health assessment at www.myactivehealth.com/conehealth.

STEP 3: Identify your current health status and complete the corresponding health action step.

Employees and spouses need to complete at least one health action step by identifying their current health status and completing the matching health action step using the tool below.

New to health insurance coverage after Jan. 2, 2020?

Keep your premiums low in 2020!

Who: Employees and spouses who are new to a Cone Health medical plan (Choice, Save, and Focus).

What: Complete a Health Assessment to earn the Healthy Lifestyle Premium rate in 2020.

By When: Within 30 days of coverage beginning on a Cone Health medical plan.

How: Create an account at www.myactivehealth.com/conehealth and complete your health assessment.

New hires or status changes (ie. new to health insurance) must complete their health assessment within 30 days of enrolling in the medical plan.* Failure to complete this step will result in a $15 per pay period increase throughout 2020.

The Healthy Lifestyle Premium Program is a voluntary wellness program for employees and spouses on the medical plan. If it is unreasonably difficult due to a medical condition for you to achieve the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, you may qualify for an opportunity to earn the same incentive through an alternative course of action, that will be developed with your doctor.
