Failed to perform basic, complete physical exam, which has occured multiple times. Seems more interested in getting to next patient. Entered onto the Visit Summary information that was the exact opposite of the information I provided him. In the past, he has also entered information onto the Summary that we did not discuss and that was inconsistent with the answer I would have given him if we had discussed the issue.

Dr. Jones was very good. Although my visit was not in his specialty, he asked quested, listened to my questions and answered so I could understand. Educational In his answers. I would consider moving my cardiac needs to him In the future.
