Sarah Elizabeth Chambers, MSW, LCSW, is a licensed clinical social worker. She practices in an integrated behavioral health setting. She specializes in behavioral health medicine, neurology and chronic disease management.
Using a holistic and integrated approach, she understands the interconnected web of physical and mental health and the importance of having practice knowledge across disciplines. Approachable and down-to-earth, she uses various lenses to conceptualize a snapshot of each patient’s unique care needs. As part of an interdisciplinary care team, she strives for a culture of excellence and provides comprehensive, up-to-date and standards-based care.
In her spare time, she enjoys being outside, spending time with her dog, growing vegetables, learning, working on creative projects, and making community connections.
