Biography
After starting her health career as a teen volunteer at Cone Health, Sarah Slack, DNP, AGNP-C, received her doctor of nursing practice degree and became a board-certified adult-gerontology primary care nurse practitioner. She specializes in geriatrics and neurology, including migraine, seizure, stroke, dementia and multiple sclerosis.
Working closely with patients and their families, she helps them achieve their health goals, especially for chronic conditions. A keen listener and compassionate educator, she is passionate about providing geriatric patients with care that is patient directed and family inclusive while promoting well-being and maximizing quality of life.
In her spare time, she enjoys spending time with her husband, daughter and 2 beagle rescue dogs. She loves exercising, including spin and barre classes.
