Patients with atrial fibrillation typically have multiple co-morbidities and are at higher risk for stroke and heart failure, resulting in recurrent admissions to the hospital. The resources at Cone Health Atrial Fibrillation Clinic at Magnolia Street are available to help co-manage patients with this condition. Collaboration with cardiology, primary care, pharmacy, nutrition and electrophysiology will provide quick access and prevent re-admissions to the hospital as well as improve quality of life.
Cone Health Atrial Fibrillation Clinic at Magnolia Street is staffed by a dedicated, multi-disciplinary team. Our experts co-manage patients with:
- New onset of atrial fibrillation
- Rate/Rhythm control
- Symptom management
- Education
- Pharmacological cardioversion
We will see patients within 3 days after discharge to assure appropriate follow-up care consistent with evidence based guidelines and serve as a resource to patients and referring providers.
