Appointment Guidelines

Bring your insurance card and all medication bottles, including prescriptions, supplements and over-

• Before your scheduled appointment with us, you will receive a reminder phone call.
• If you’re a new patient, please plan to arrive 15 minutes early for your appointment. Bring your insurance card, a list of your current medications (with dosage), a list of any relevant previous medical records and any other pertinent information (such as test results, X-ray films from another provider, etc.).
• If you need to cancel or reschedule your appointment, please let us know as soon as possible.

After-Hours Calls

• We have providers on call after office hours and on weekends. They can be reached by calling our main office number – (336) 663-5700.
• All prescription refill requests and/or medication related questions, should be handled during office hours. No refill requests will be given after hours or on weekends.

Payment Information

• You are expected to pay in full or pay your co-payment for all office services at the time of your appointment. If you are not able to make your payment at the time of your visit, please call the office to make other arrangements.

• If you have a managed care insurance plan that requires referral authorization from your primary care physician, you are responsible for bringing that referral with you from your primary care physician. If you do not obtain a referral authorization from your primary care physician, you may be responsible for the entire bill.

• For our Medicare patients, our physicians accept "assignment" which means Medicare will pay us directly for 80% of the Medicare "allowable" charges. The patient is responsible to pay their annual deductible and 20% of allowable charges (or the co-payment). If a Medicare patient has supplemental insurance, we will file this insurance after receiving payment from Medicare. The patient is still responsible for the annual deductible. If Medicare is the secondary insurance, we will file it after we receive payment from the primary insurance.

• If you have any disability, FMLA, return to work notices or any other forms, we will be happy to assist you in filling them out. There is a $29 fee for filling out disability forms. Please allow us 10 to 14 business days if any forms need to be filled out.
