A sudden loss of independence challenges the body and spirit. For more than 40 years, the Cone Health Inpatient Rehabilitation Center has helped patients and families return to full, productive lives after experiencing the effects of a debilitating illness or injury.
Cone Health maintains accreditation from The Joint Commission. The Inpatient Rehabilitation Center is accredited by the Commission on Accreditation for Rehabilitation Facilities (CARF) for three programs:
We provide specialized rehabilitation services as well as access to critical care. Our center is staffed by an experienced, certified and dedicated team of physicians and rehabilitation professionals who provide personalized, comprehensive and fully integrated care. The Inpatient Rehab Center is also part of a continuum of care that includes a network of outpatient rehabilitation centers and a variety of community support groups.

Individualized Care Plans
Collaborate with your rehabilitation care team to set goals and create a customized care plan. You’ll not only develop skills and strategies for daily living but also improve your ability to participate in community and leisure activities.
Depending on your needs, your care plan may incorporate:
- Physical therapy
- Occupational therapy
- Speech therapy
- Neuropsychology or psychology services
- Recreation therapy
- Pet therapy
- Nutrition management
- Spiritual care services
- Therapeutic community outings
Rehabilitation Nursing Care
You’ll feel comfortable, safe and well cared for at the Cone Health Rehabilitation Center at Moses Cone Hospital. We practice hourly rounding, which means a nurse or nursing assistant regularly comes to your room to see how you’re feeling and how to meet your needs. Evidence shows this practice can reduce fall rates and pressure ulcers, as well as increase patient satisfaction.
Family Involvement
Your family and caregivers are an important part of your rehabilitation team. They’re encouraged to visit often, join you for therapy sessions and take part in family conferences when needed.
Home Modifications
We offer recommendations and suggestions for home modifications based on information provided by the patient and family to facilitate a safe discharge.
Follow-Up Care & Services
Expect a phone call from us within a week after leaving the rehabilitation center. We’ll ask about how you’re doing and whether we can help you connect with any services or resources that may benefit you.
If you need to continue therapy after returning home, ask for a referral to Cone Health’s outpatient physical therapy, occupational therapy or speech therapy services at a location near you.
Meet the Team
Your team of specialty skilled and highly trained professionals will work with you and your family throughout your rehabilitation stay. Many are certified rehabilitation nurses and brain injury certified rehabilitation professionals.
- Physiatrists are physicians specially trained in physical medicine and rehabilitation. They guide you on the journey to recovery. They are responsible for the overall management of your rehabilitation program. A physiatrist or covering physician will visit you daily.
- Physician Assistants work closely with physiatrists to provide 24-hour coverage of your medical care.
- Rehabilitation Nurses, many of whom are Certified Rehabilitation Nurses, provide 24-hour care and monitoring. Your nurse will create a personalized care plan based on your goals and will teach you and your family to become proficient in handling your needs.
- Physical Therapists help you improve movement, strength, balance, coordination and endurance.
- Occupational Therapists focus on improving coordination, thinking skills and daily living skills such as bathing, dressing, grooming, eating and homemaking.
- Speech-Language Pathologists help you improve communication, swallowing and thinking skills.
- Recreational Therapists use leisure and music activities to improve movement, coordination and thinking skills. They also arrange for community re-entry activities and pet therapy.
- Dieticians offer nutritional care and counseling to aid in the recovery process.
- Neuropsychologists provides psychological testing, treatment and counseling to help you and your family adjust to changes that occur with illness and injury.
- Social Workers help involve you and your family in decisions about the rehabilitation program. They coordinate rehabilitation services to meet your needs and help plan for any home needs, such as follow-up nursing care and therapy services. They can help you and your family understand insurance information and explain options for receiving financial assistance. Social workers may also provide counseling services.
- Chaplains provide spiritual and emotional care as requested.
