Rehabilitation Center Admission
Admission to the Cone Health Rehabilitation Center at Moses Cone Hospital is based upon your need for, ability to participate in and benefit from our comprehensive inpatient rehabilitation services. Our goal is to help you return to your home and community or prepare you for the next level of care.
To be admitted to the Cone Health Inpatient Rehabilitation Center, patients must meet the following criteria:
- Need close medical supervision by a physician who specializes in rehabilitation medicine
- Be medically able to participate in rehabilitation activities
- Require intensive rehabilitation services for problems in two or more of the following areas: self-care, mobility, cognition/communication.
- Need 24-hour rehabilitation nursing care and at least two of the following: physical therapy, occupational therapy, and speech therapy.
- Have the potential to participate in at least three hours of therapy per day, five days a week.
- Be likely to benefit significantly from the program within a reasonable period of time.
- Have an identified preliminary discharge plan.
- Be 18 years of age or older.
- Be free from any major psychiatric or behavior disorder
