Family Medicine Center Earns Recognition as Patient-Centered Medical Home

The National Committee for Quality Assurance has recognized the Cone Health Family Medicine Center as a Level 3 Patient-Centered Medical Home.

This designation indicates that the Family Medicine Center offers a strong physician-patient relationship and has replaced walk-in visits and missed appointments with a long-term continuity of care that is based on mutual goals of health and wellness.

Physician practices must apply for the recognition. The practices are then evaluated on areas, including access and communication, patient tracking and registry functions, care management, electronic prescribing and others.

Kathy Foster, RN, led the application efforts. A team including Pete Koval, Pharmacist; Lee Chambliss, MD, Medical Director; Denya Hawkins, Director, Family Medicine Center; Ruth Sappenfield, Application Analyst, Cone HealthLink; Mari Hawkins, Secretary; and the entire residency program helped achieve this important recognition.

“We expect this will help convey to patients that we place their access to care as a top priority,” Foster says. “It also shows that we use the most up-to-date technology, practice evidence-based medicine, value wellness and prevention, and strive to establish long-term relationships between patients and physicians,” Foster says. “Staff will know they are members of a healthcare team that has been recognized as a model of best practice.”

Chambliss adds, “For physicians, it is an acknowledgement of our hard work toward meeting national quality of care goals. It is also an incentive to continue to strive to improve our patients’ care.”

The recognition marks the culmination of years of work, says Bill Hensel, MD, Director, Moses Cone Family Medicine Residency Program.

“The quality efforts and the processes and procedures that our office refined in preparation for this application fit perfectly with Cone Health’s Triple Aim goals and with our community’s clinical integration efforts,” Hensel says. “I am confident that the lessons we learned will help others moving forward.”
