Triad HealthCare Network Enters into Value-Based Agreement with Humana

Triad HealthCare Network, one of the top performing ACOs in the country and Humana Inc. (NYSE: HUM), a leading health and well-being company, announce a new Accountable Care agreement covering members in Guilford, Randolph, Rockingham and Alamance counties. 

The new Humana-Triad HealthCare network accountable care agreement features incentives tied to performance improvement, and will bring better patient care coordination and a world-class patient experience to Humana members in North Carolina. The arrangement will offer more personalized care and a broad range of population management tools such as predictive analytics and wellness programs focusing on chronic conditions and disease management.

“Triad HealthCare Network believes that by providing the right care in the right place at the right time, we keep our patients healthier and reduce the costs of their care,” says Triad HealthCare Network Vice President and Executive Director Steve Neorr. “Partnering with like-minded organizations such as Humana allows us to bring better care to more people while being more cost effective with health care dollars.” 

The value-based arrangement emphasizes quality outcomes with a focus on care coordination and uses measures as defined by the National Committee for Quality Assurance Healthcare Effectiveness Data and Information Set. These measures set standards for diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and dozens of other illnesses.

Under the terms of the accountable care agreement, Humana will offer health plans featuring Triad HealthCare Network physicians as well as Cone Health’s six hospitals and ancillary facilities in the Piedmont
