Earn Patient-Centered Medical Home Designations
All seven primary care locations of LeBauer HealthCare have been recognized by the National Committee for Quality Assurance (NCQA) as level-3 Patient-Centered Medical Homes™. Patients can depend on Patient-Centered Medical Homes for comprehensive medical management, from referrals and testing to hospitalization and more. The recognition was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts.
The NCQA Patient-Centered Medical Home standards emphasize the use of systematic,coordinated care that supports access, communication and patient involvement. “We work very hard at LeBauer HealthCare to create long-term relationships with our patients and to use the latest scientifically-proven ways to care for them,” says Cone Health Medical Group Primary Care Director Brian Hunt. “This recognition confirms our focus on every patient, every day at every LeBauer HealthCare primary care office.”

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics and American Osteopathic Association, and others.
