Innovation at Cone Health Leads to Better Health at Lower Costs

WellsmithThrough innovation, Cone Health provides value-based health care to the people we serve. This means better health at lower costs.

One of those ways is called, WellSmith. Wellsmith helps diabetics control their disease with their smartphone. The program connects a smartphone with blue-tooth connected devices, like a scale and glucose monitor. The patient gets reminders through the app and a care teamhelps them meet their goals.

Vicki, a Cone Health employee, was in the pilot program for WellSmith. She has been able to keep her diabetes under control with the program. This has meant fewer trips to the doctor and a more active lifestyle for Vicki.

The North Carolina Hospital Association (NCHA) produced a video telling Vicki's story. They are using it to promote the success of value-based care in North Carolina.
