- Cardiometabolic Screenings (hypertension/blood pressure monitoring, diabetes/blood glucose/AIC testing, high cholesterol/lipid screening, obesity and BMI assessment)
- Early Detection & Risk Identification (identification of patients at risk for heart disease and stroke, comprehensive cardiometabolic risk assessment)
- Chronic Disease Management (ongoing monitoring of blood pressure, glucose, cholesterol, weight and support for existing cardiometabolic conditions)
- Health Education and Lifestyle Counseling (nutrition and physical activity guidance, medication adherence education, personalized risk reduction strategies)
- Care Coordination & Follow-Up (referrals to primary care and specialists, navigation support for continued treatment, coordinated follow-up to ensure long-term management)
