Return the form by mail, email or return it to your provider’s office. Access requests for patients with diminished capacity should contact the MyChart Help Desk at 336-832-4278 to learn how to get full access.
*In North Carolina, minors aged 12 or older can legally consent to certain services without parental permission, with limited parental access granted through proxy authorization.
