Team Member Eligibility
To be eligible to participate in Cone Health benefit programs, you must be a full-time or part-time employee as defined in the Cone Health Employment Policy. Upon hire, your position or expected hours or service will determine if you are eligible to participate in benefit programs.
- Full-time/Part-time team members (FTE .5-1.0)- eligible upon hire; must choose benefits within 31 days of hire date.
- Eligible dependents - includes team member's spouse and children up to age 26, plus dependent children of any age who are physically or mentally unable to care for themselves.
You will need to provide proof of eligibility for your dependents, such as a copy of a birth certificate, marriage license, court orders or any other qualifying legal document.
Team members with employment status less than a .5 FTE are not eligible to participate in Cone Health benefit programs.
If you are in a non-benefits eligible status, eligibility is determined using a look- back measurement method. The look-back measurement method is based on IRS final regulations under the Affordable Care Act (ACA). Its purpose is to provide greater predictability for eligibility determinations. Cone Health uses a 12-month look-back measurement method to determine who is a full-time employee for the purposes of ACA benefits eligibility.
