LifeLifeWell employee wellness offers a variety of programs to support employees in managing health conditions such as diabetes, COPD, and asthma. Additionally, we provide resources and support for employees during significant life stages, including pregnancy, to ensure their well-being and health.
Diabetes Condition Management
We are proud to offer this unique benefit to our Cone Health medical plan members with diabetes. Participants in the Diabetes Condition Management participants are eligible for a Diabetes Pharmacy Benefit and Well-being Bucks. Active Health Management is our partner that provides diabetes management through Certified Diabetes Educators and Registered Nurse Coaches. You can enroll anytime by contacting their team at 1-855-294-6577 or setting up a coaching appointment through your ActiveHealth online wellness account.

As part of the program, you'll receive helpful information about prenatal care, early labor symptoms, what to expect before and after delivery, newborn care, and more.

You can enroll anytime by contacting their team at 1-855-294-6577 or setting up a coaching appointment through yourActiveHealth online wellness account. Employees on a Cone Health medical plan who complete three (3) coaching calls are eligible for Well-being Bucks!
