Employees (and spouses) need to complete the Healthy Premium requirements every year to keep your premiums low. By completing these requirements, you are eligible for a $520 ($20/PP) discount in 2027.

How: Create an account at www.myactivehealth.com/wellbeing or though the MyActiveHealth Wellness app and complete your health assessment. Your annual physical will automatically upload into your ActiveHealth account within 6-8 weeks. You do not need to wait until your annual physical to complete your Health Assessment.

Who: All employees and spouses on a Cone Health medical plan on January 1, 2026.

What:

• Get an annual physical between January 1, 2025, and September 1, 2026.
• Complete your health assessment by September 1, 2026.

Were you hired or enrolled in benefits in 2026?

You automatically get the discount but will need to complete your Healthy Premium requirements the following year in order to keep your premiums discount.

Earning the Healthy Premium rate is voluntary for employees and spouses on the medical plan. If it is unreasonably difficult due to a medical condition for you to achieve the standards for the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, you may qualify for an opportunity to earn the same incentive through an alternative course of action, that will be developed with your doctor.
