Vaccination Schedule - Birth to Age 2

Birth: Hepatitis B (HepB) #1

1 Month:

2 Month: Vaxelis (DtaP, Hib, Polio, HepB), Prevnar (Pneumococcal vaccine), Rotateq (oral vaccine, rota virus vaccine)

4 Month: Vaxelis (DtaP, Hib, Polio, HepB), Prevnar (Pneumococcal vaccine), Rotateq (oral vaccine, rota virus vaccine)

6 Month: Vaxelis (DtaP, Hib, Polio, HepB), Prevnar (Pneumococcal vaccine), Rotateq (oral vaccine, rota virus vaccine)

9 Month: 

12 Month: Hepatitis A (HepA), Varicella (chicken pox), MMR (measles, mumps, and rubella)

15 Month: Pentacel (DtaP, Hib, Polio), Prevnar

18 Month: HepA

Vaccination Schedule - Ages 4-12

4-5 years: Quadracil (Dtap, Polio), ProQuad (MMR , Varicella)

9-12 years: HPV (human papilloma virus)- recommended by the American Academy of Pediatrics and American Cancer Society. Series of 2 vaccines with at least 6 months between the first and second vaccine.

11-12 years (prior to the start of 7th grade): Tdap, Meningococcal (ACWY)

Vaccination Schedule - Teen

16-18 years: Meningococcal (ACWY), Meningococcal B (recommended)
