Chronic Care Management

If you are a Medicare patient who suffers from two or more chronic conditions, you might be eligible for Chronic Care Management (CCM) at Cone Health, which gives you access to a broad level of personalized support. This is a program built for patients managing 2+ diseases like arthritis, diabetes, depression, high blood pressure, etc. who would benefit from additional support.

You’ll receive a personalized care plan as well as access to a team of dedicated health care professionals who can help you plan for better health and stay on track for good health, too. With CCM, you’ll receive much more communication and support from Cone Health between your regular appointments, helping you feel more connected and less alone.

Our Services

Qualifying patients will have access to the below services through CCM:

At least 20 minutes a month of chronic care management services

• Personalized assistance from a dedicated health care professional who will work with you to create your care plan
• Coordination of care between your doctors, specialists, pharmacy, testing centers, hospitals and more
• Phone check-ins between visits to keep you on track
• 24/7 emergency access to a health care professional
• Expert assistance with setting and meeting your health goals

As part of CCM, you’ll be able to work with the following team members, based on your needs:

• Care Guide: community resource connection
• Pharmacist: medication and disease management
• Care Manager: disease management, care coordination and complex case management
• Clinical Social Work: Complex Care Coordination, level of care concerns, mental health care coordination and assistance with long-term mental health connections

You’ll be able to meet with these team members either at your existing provider’s office or in the comfort of your own home via a phone or video conversation.
