If you’re a Cone Health patient at a participating practice or Triad Health Network patient who needs assistance getting past a hurtle related to your health or lifestyle, our team of Community Resource Care Guides are standing by to help you!
Any Cone Health provider at a practice who participates in our program can refer a patient to a Care Guide, and our team works to connect patients to community resources that can help make a patient’s life and health better.
Our team works to support other facets of Cone Health’s extension of care programs, including Chronic Care Management and Care Coordination, but we also work with patients on a standalone basis as needed. We are a non-clinical team, so we offer support that does not involve things like medication management (although we can help connect you with another ECC program that CAN assist with those things!).
Community Resource Care Guides are able to assist any Cone Health patient, regardless of if they are covered by insurance.
Our Services
What does a Care Guide assist with? We connect patients with a broad variety of non-clinical support, including:
- Financial support for things like medical equipment, covering a surgery, paying a light or heat bill, buying a CPAP machine and much, much more
- Transportation barriers keeping them from making their Cone Health appointments and having access to other lifestyle and care needs
- Smoking cessation
- Support for caregivers
- Assistance for patients who are homeless
